Our long-term care facility located in the Upper Bronx and Westchester areas are looking for experienced LPNs
* Orientation will be in the next two weeks of October
* Credentials needed as follows:
(1) SS card
(2) State-issued ID
(3) Work eligibility document (US passport, birth cert, Permanent Resident card, EAD)
(4) LPN license and registration
(5) physical report valid during the last 10 months (including PPD, if positive with chest x-ray result; and MMR titer level lab results)
(6) two reference letters
